Admission and Registration Information
Instructional Information for LRMC Patients
Upon your arrival to the hospital, all patients (whether Inpatient or Outpatient ) must first go to Registration for instructions.
If you are at the hospital due to an Emergency, or an Outpatient test or procedure, a clerk will process your registration and send you to the appropriate area.
If you already know you are to be admitted, please go to the Admitting Office which is located beside the Registration area. The admitting clerk will process your admission, send you for any additional testing you might need, and then send you to your hospital room. However, some patients may possibly be scheduled for a procedure, such as surgery, before being taken to their hospital room.
Please feel free to ask any questions of the Registration or Admitting personnel.
Scheduled Inpatient Admission
- On day of admission, please bring comfortable clothing and shoes.
- Bring personal hygiene products.
- Bring a list of medicines with dosage.
- Bring your insurance card(s).
- If a deposit is required, please bring it at time of admission.
- Please do not bring valuables
Outpatient Surgery Instructions
If patient is having an outpatient surgical procedure, the patient is to follow the doctor's instructions including having lab work and/or x-ray within the time frame ordered by the doctor. The patient is also to go to the outpatient surgery department (2nd floor) between 8 a.m. and 3 p.m. to receive instructions on what to expect the day of surgery. Prior to having any surgical procedure, the patient is to follow the doctor's written instructions and to ask any questions or make known any concerns the patient may have prior to the surgery.
What to Bring for Outpatient Testing or Procedures
Please bring physician order, make sure diagnosis is on form before leaving doctor's office.
Please bring a photo ID, if available, insurance cards and a list of any and all medications with the dosage. If the need arises, the medical personnel are aware of all medications the patient is currently taking.
If you are having a scheduled test, patients can pre-register in their doctor's office or at the main registration department at the hospital prior to having scheduled test(s) or procedure(s). However, the patient must stop by registration, at the main hospital, to complete their registration on the day of their test(s) or procedure(s). The patient must also have made payment arrangements prior to arrival for scheduled test(s) or procedure(s).
All payment arrangements must be made prior to testing. However, payments and or deductibles can be made at time of registration. If you have questions about your deductibles or payment arrangements, please call 304-831-1185 Monday through Friday between the hours of 8:30 a.m. to 5:00 p.m. and 6:00 p.m. to 11:00 p.m.
All patients should arrive at the hospital at least 30-45 minutes prior to all scheduled tests.
